Position Summary: Responsible for daily function of drug distribution systems, inventory control, drug information and education, departmental performance improvement and medication use activities.
Position Description: 1. Complies with hospital/departmental policies and procedures, i.e., attendance, mandatory education, safety, infection control, etc.
Assists in coordinating and supervising departmental functions. • Takes responsibility to see that required daily functions are completed when working as "MAIN" pharmacist (drug purchases, drug charges, floorstock replenishment, etc.). • Supervises the pharmacy support staff in a consistent, fair manner and leads by a professional example. • Effectively communicates important issues with oncoming shift. • Resolves conflicts professionally. • Maintains departmental adherence to state and federal law and professional standards.
Accurately and effectively provides medication therapy. • Prioritizes work. • Appropriately reviews physician order(s) and inputs into the pharmacy profile system. • Reviews drug regimen for drug allergies, rational therapy, contradictions, reasonable dose and route, reasonable directions, duplication of therapy, drug-drug interactions, other interactions, and adverse drug reactions. • Applies knowledge of pharmacotherapy to identify patient and family requirements relative to age-specific needs (see age-specific criteria). • Communicates with other disciplines and the patient to resolve issues. • Appropriately check all medications prior to dispensing.
Actively participates in intervention, performance improvement and professional programs. • Makes or recommends appropriate therapeutic interchanges. • Reports Adverse Drug Reactions and Medication Errors. • Provides consults upon request. • Provides patient medication counseling upon request. • Provides drug information upon request. • Assists in training, educational, and quality assurance programs. • Serves on hospital teams, as directed. • Provides medication in-services, as directed. • Completes special projects as assigned by Director. • Offers constructive recommendations for improving departmental performance.

Essential Responsibilities: 1.0 Assures customer/personal safety by maintaining a safe and therapeutic environment and by adhering to Employee Health and Safety program. 1.1 Utilizes safety guidelines, material safety data sheets (MSDS), and reports hazards, accidents, and unsafe equipment and/or practices. 1.2 Demonstrates adherence to universal precautions and aseptic technique. 1.3 Verbalizes and/or demonstrates roles and duties in fire, evacuation, and disaster drills. 1.4 Regularly reviews hospital information and/or attends educational activities related to safety and injury prevention.
2.0 Maintains and increases standards of performance and education. 2.1 Takes initiative to increase job knowledge through additional training, cross training skills development, and/to continuing education. 2.2 Attends or fulfills requirements for all mandatory hospital inservices to include annual education, safety update and training by anniversary date. 2.3 Reviews all new and revised policies and procedures or designated manuals. 2.4 Attends at least 80% of department meetings and/or verifies acknowledgement by reading meeting minutes.
3.0 Ensures accurate and efficient drug distribution services. 3.1 Verify new orders against profile for allergies, interactions, duplication and ensure patient weights and allergies are documented on profile. 3.2 Orders are processed within 1 hour of receipt or before next administration time. 3.3 No more than 1 dispensing error of commission per month. 3.4 No more than 2 dispensing errors of commission (original order) per month. 3.5 Ensure accuracy of medication order dose instructions, par level, label comments, and container labels. 3.6 Supervise all delegated acts related to drug distribution by supportive personnel. 3.7 Run daily computer reports and MARS. 3.8 Maintain appropriate pre-packing and compounding records. 3.9 Enter IV orders for labels, prepare, deliver, and check profiling and preparation of sterile products on a daily basis. 3.10 Follow procedural guidelines for preparation of all sterile products.
4.0 Provide for control and distribution of controlled substances. 4.1 Adhere to procedural guidelines 100% for CSAR log and control drug receipt. 4.2 Maintain perpetual inventory of C II drugs and conducts monthly audits as assigned.
5.0 Interprets, prepares, and dispenses outpatient med orders, and maintains appropriate records. 5.1 Maintain outpatient prescription files per State Board regulations. 5.2 Document reconciliation of ER take home inventory weekly as assigned.
6.0 Demonstrates ability to select, utilize supplies and equipment in a cost effective manner, and to reduce waste. 6.1 Ensure contract items are ordered through online order entry system. 6.2 Follow procedures for reconciling Accunet exceptions list. 6.3 Transmit wholesale order daily as scheduled.
7.0 Demonstrates an ongoing awareness of; and participates in departmental and hospital performance improvement plans. 7.1 Participates on PI teams as assigned. 7.2 Reviews MAR variances, reconciles discrepancies, and clearly documents reasons for variances on Pharmacy copy of MAR. 7.3 Conducts analysis and summarizes results as assigned.
8.0 Serves as a drug information resource and provides clinical pharmacy services to hospital staff and patients.
8.1 Conducts patient education counseling as requested by physician, other healthcare providers, or based on professional judgement. 8.2 Conducts inservices/orientations for nursing and medical staffs as assigned. 8.3 Documents all clinical interventions and identified prescribing deficiencies in pharmacy computer module
__Education and Experience: Required: *Bachelor of Science in Pharmacy from an accredited college or university; Two years hospital pharmacy experience *Preferred: PharmD from an accredited college or university; Experience in NICU, PEDS, ED, Transplant and Main Hospital __* Knowledge, Skills, and Abilities: * Ability to communicate clearly and effectively; excellent oral and written communication skills; ability to relate to diverse populations in an open, friendly, and accepting manner; ability to make timely and sound decisions under stressful conditions. Management of conflicting priorities; problem solving, team building; comfort level with ambiguity and autonomy; ability to form effective working relationships to achieve established goals and objectives. Computer/data entry skills. Adherence to customer service behavioral standards. Must be able to identify medication use safety issues and intervene in professional manner with nursing and medical staffs. Must be able to demonstrate understanding of HCA's and St. David's "Patients First" safety initiative by strict compliance to all safety protocols and procedures.
