The West Texas VA Health Care System (WTVAHCS) provides a variety of patient care services, with state-of-the-art technology as well as offering numerous educational programs. Outpatient services include: Ambulatory Care Service, Mental Health, Women's Health, Transition and Care Management, Home Based Primary Care, Tele-Health, Anticoagulation Clinics, Ophthalmology, and Dentistry. Skilled nursing care and acute rehabilitation is provided in the Community Living Center, with specialty referrals to other VISN 17 facilities and community services. Staffed with approximately 600 employees, the facility serves counties in West Texas and New Mexico area, with a Veteran population of approximately 57,000. The WTVAHCS is part of the VA Heart of Texas Health Care Network, VISN 17 and is centrally located at the crossroads of Interstate 20 and Highway 87 in Big Spring, Texas with Community Based Outpatient Clinics (CBOC's) located in Abilene, Odessa, San Angelo, Ft. Stockton, Texas and Hobbs, New Mexico.
Big Spring, "The Crossroads of West Texas" is home of the newly restored historic "Settles Hotel", "The Spring", and home of the National Championship Howard College Hawks Basketball and Baseball teams. The West Texas VA Health Care System, Big Spring, Texas is currently recruiting for the position of Nurse Practitioner/Physician Assistant for the Home Based Primary Care Program. This position may be located based in Big Spring and/or Odessa, Texas.
The Nurse Practitioner/Physician Assistant will function as a primary care provider at the patients' home setting. He/she will be responsible for the initial evaluation of a broad range of assessments; diagnosis; treatments; ordering diagnostic & therapeutic tests and studies; initiating consults; referrals and member of a interdisciplinary team working collaboratively with other members of the health care team and provide additional activities in the area of education and quality improvement. The Nurse Practitioner/Physician Assistant will guide, document, and appraise the performance of others, as well as identify, analyze and resolve problems that impede the delivery of patient care.
Incumbent must be able to travel within the catchment area during his/her tour of duty. Incumbent may be required to staff an HBPC clinic. One-two years experience as an Advanced Registered Nurse Practitioner and/or Physician Assistant in Adult Health or as a Geriatric Practitioner-Prescriptive Authority is highly preferred.
He/she will consult with the supervising physician as needed; communicate relevant patient data in case presentations and medical records; formulate, initiate and evaluate health care teaching related to patients and families; facilitate communication related to patients and consult to nursing and medical staff.

QUALIFICATION STANDARDS FOR NURSE PRACTITIONER:
Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing.
Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
QUALIFICATIONS FOR PHYSICIAN ASSISTANT:
(1) Bachelor's degree from a PA training program which is certified by the Accreditation Review Commission Education of the Physician Assistant (ARC-PA); or
(2) Graduation from a PA training program of a least 12 months duration which is certified by the ARC-PA and a bachelor's degree in a health care occupation or a health-related science; or
(3) Graduation from a PA training program of at least 12 months duration which is certified by the ARC-PA and a period of progressively responsible health care experience such as an independent duty medical corpsman, licensed practical nurse, registered nurse, medical technologist, or medical technician. The duration of approved academic training and health care experience must total at least 5 years.
Certification - Must possess certification by the National Commission on Certification of Physician's Assistants (NCCPA).
Preferred Experience: One-two years experience as an Advanced Registered Nurse Practitioner and/or Physician Assistant in Adult Health or as Geriatric Practitioner-Prescriptive Authority is highly preferred.

Physical Requirements: This position requires moderate lifting, 15-45 pounds, light carrying, under 15 pounds, straight pulling and pushing, reaching above shoulder, use of fingers, both hands required, walking, standing, kneeling; repeated bending, near vision correctable at 13" to 16" to Jaeger 1 to 4, hearing (aid permitted), and clear speech.
