The Patient Care Assistant under supervision of Registered Nurse (RN) or Physician, provides direct patient care by assisting RN in implementing plan of care to meet patient and family needs. Maintains clean and orderly work and patient care environment. Assists in patient care, including examinations and procedures. Obtains and records patient vital signs, heights, weights, intake and output, and reports results and observations. Assists patients with bathing and feeding.


Assists the Registered Nurse and Physician with implementing patient plan of care. Collects and records patient data, reporting information and observations to Nursing Personnel. Obtains and records patient vital signs, height and weight, Lab and diagnostic results, estimates nutritional intake and output, performs DFS, maintains and administers Foley Catheter care and assists attending Nurse in department

Assists patients with orientation of patients to Hospital room and bed. Assists patients with dressing and undressing, personal bathing and oral hygiene, changing linen, getting in and out of bed, ambulation, etc. Prepares patients for meals and assists patients with eating, if required. Collects specimens to include urine, stool, sputum, etc. May transport patients

Performs Unit support duties as directed by Physician or RN. Attends in-services, Unit meetings and conferences. Runs errands, picks up supplies, stocks crash cart, unpacks and stores supplies. Transports specimens and receives and transports patients, etc

May assist with completing and maintaining Unit and patient records, files, and forms associated with patient admission, transfer, discharge, procedures, and patient care. Answers telephone, transcribes Physician's Orders, requests supplies and equipment, accesses and records patient data into computerized patient record

Answers and responds to patient call lights and requests

Complies with Baylor Scott & White Health policies and procedures, adheres to safety guidelines and ensures a safe work and patient care environment. Observes proper body mechanics while performing work area duties and patient care

Follows the plan of care which includes the age and/or development stage of the patient

Minimum/No Experience

H.S. Diploma/GED Equivalent

Basic Life Support

Verbal and written communication skills

Ability to demonstrate accuracy and attention to detail
