Licensure includes RN and LPN/LVN, depending on grade level, with current unrestricted licensure in applicable state.
These roles identify, coordinate, or provide appropriate levels of care under the direct supervision of an RN or MD.
Function is responsible for clinical operations and medical management activities across the continuum of care (assessing, planning, implementing, coordinating, monitoring and evaluating).
This includes case management, coordination of care, and medical management consulting.
Function may also be responsible for providing health education, coaching and treatment decision support for members.
Includes Health Coach, Health Educator, and Health Advocate roles that require an RN.Primary Responsibilities: Send and receive daily office communications to be faxed, emailed and entered into case monitoring and tracking tools Maintain and update cases through internal system, IQ Receive, update and monitor claims via ISET and Impact PRO, based on client, provider and business unit High telephonic proficiency, with a focus to meet daily assigned contact metrics Must have a dedicated home office space to complete daily tasks and clients phone contacts Must be able to work 40 hours per week between the hours of 8am – 8pm; with 3 weekly last stays between the hours of 5pm – 8pm Ability to travel up to approximately 5%, if needed Requires current unrestricted LPN licensure in the state in which you reside Required Qualifications: High School Diploma/GED Unrestricted LPN License in the State of residence Ability to travel up to approximately 5%, if needed Must be able to work 40 hours per week between the hours of 8am – 8pm; with 3 weekly late stays between the hours of 5pm – 8pm Must be willing to obtain additional Licensing/Certifications as needed 1+ years of Clinical LPN experience Must be able to create, edit, copy, save and send data utilizing Word, Excel and Outlook Must be able to navigate a PC to open applications, send emails, and conduct data entry Preferred Qualifications: Bilingual fluency Any kind of CKD (Chronic Kidney Disease) knowledge or experience Any kind of Cardiac or Diabetic knowledge or experience Experience with IQ, My Health Direct, ISET, Impact PRO Soft Skills: Excellent communication skills Ability to effectively communicate products clearly and accurately Careers with Optum.
