Texas Health Fort Worth is currently hiring a full time Diabetes Educator RD.

Degree of BS required and one of the following advanced practice certifications, if practicing

within the certification: CDE (National Certification Board of Diabetes Educators (NCBDE):

CNSD (National Board of Nutritional Support Certification)); CSP (Certified Specialists in

Pediatric Nutrition);Commission on Dietetic Registration; CSR (Certified Specialist in Renal

Nutrition) Commission on Dietetic Registration; CEDS (Certified Eating Disorder Counselor;

(Other certifications must be approved by THR Food and Nutrition Council) OR Masters Degree

in Nutrition, Food Service or Related Field.

3 years as a Registered Dietitian; 2 years as Certified Diabetes Educator preferred

Registered with CDR-Licensed in State of Texas (TSBED), BLS upon hire and must be maintained. Certified Diabetes Educator (CDE) by the NCBDE (National Certification Board for Diabetes Educators), or achievement of CDE within 6 months of meeting the experiential requirements for application for certification. If non-CDE, must meet annual continuing education requirements by the American Diabetes Association for meeting National Standards for Diabetes Self-Management Education and Support.

Effective and positive interpersonal skills. Ability to communicate information to a culturally and socioeconomically diverse population, maintain a high degree of confidentiality, and ability to function in a fast paced environment. Computer skills, time management skills and advanced knowledge of current clinical practice recommendations, standards of care, health issues and professional practice related to diabetes.

Demonstrates advanced knowledge and clinical diabetes expertise serving as a clinical expert and

resource for others.

Provides consultations, demonstrations or instructions related to staff development for improved patient education. Provides consultation to professional colleagues, patients, and families regarding effective self-management skills, treatment plans, etc.

Promotes/facilitates the advancement of diabetes information for staff. Identifies learning opportunities specific to the individual or the group needs of patients or staff; plans

and provides formal and informal education to meet those needs; utilizes appropriate educational

resources that are culturally sensitive and meets the learning needs of patients/families. Education programs

reflect current, evidence-based information and pertinent research findings as indicated.

Actively participates in interdisciplinary patient care conferences contributing to the treatment plans of

patients as indicated

May participate in multi-disciplinary teams as assigned based on Wagner's Model for standard screening and treatment protocols.

Maybe tasks by leader to partner in developing relationships with local physicians and healthcare agencies to

facilitate patient access to services and resources needed to support clinical care for management of chronic conditions, etc.

Documents services rendered within the medical record reflecting thorough and timely record keeping same

day as service provided.

Monitors patient evaluation and treatment plans for compliance to national guidelines

and best practices; shares results through the Performance Improvement Committee and interdisciplinary

team rounds as indicated.

Participates in creating and maintaining databases for tracking patients enrolled in self-management programs; identifies, monitors and documents key outcome and compliance data for diabetic patients following discharge from the Emergency Department in concert with other colleagues.

In some locations may support the five year Delivery System Reform Incentive Program (DSRIP) Plan and provides reporting/feedback based on the progress of the development and implementation of initiatives outlined within the Plan. Those initiatives

consist of working with the underserved in management of chronic diabetes.

Provides feedback to community providers as needed.

Uses advanced interviewing and assessment techniques to obtain patient biophysical, psychosocial and self-care education and discharge needs of those diabetic patients identified at risk and/or underserved using the Emergency Department for chronic management of their diabetes. In doing so, works closely with Emergency Department Navigator and Transitions Team members.

Integrates appropriate assessment of the patient's readiness to learn, barriers to learning (illiteracy, language, need for communication aids in the geriatric population, etc.), preferred learning methods, cultural influences, age appropriateness, etc. into the individualized plan for the patient. Educate and assist diabetes patients through appropriate evaluation and teaching methods.

Identifies specific, individualized diabetes self-management goals; plans and implements education and behavioral interventions directed toward helping the individual achieve the identified goals; develops teaching plans reflecting acceptable standards, monitors for compliance and attainment of goals intervening as needed, and, maintains accurate documentation records related to all education encounters.

Identify methods for developing new tools to enhance instruction methods both in the Emergency department and in the community.

Utilizes effective interpersonal and communication skills to achieve desirable outcomes and responses.

Develops relationships with varied community agencies, healthcare resources and physician providers to support the varied needs of the identified diabetic patient population.

Is actively engaged in community events affiliated with THR as needed to facilitate visibility and health promotion related to the management of diabetes.

Collaborates with all customer groups (patients, department/hospital staff, physicians and other

customer groups), to meet and exceed needs/expectations.

Creates and maintains a collaborative work environment (patient, physicians and key stakeholders).

Actively engages customer groups on a regular basis to promote effective customer relationships

and communication with prompt resolution of issues and concerns.

Participates in hospital-wide projects/committees/workshops to enhance care delivery and the overall

experience of the patient.

Provides stewardship in the utilization of resources; contributes to the operational and strategic

planning for the diabetic population

Collaborates with multidisciplinary team to meet DSRIP goals.

Performs Medical Nutrition Therapy to age appropriate patients in a quality and timely manner.
