Capstone Corporation Recovery Care Coordinator in San Antonio, Texas
Capstone Corporation is currently seeking a Recovery Care Coordinator to support Commander, Navy Installations Command (CNIC) located in San Antonio, Texas.
Recovery Care Coordinators support the US Navy Wounded Warrior - Safe Harbor Program.
Specific responsibilities include:
Assists the recovering service members back to full duty or civilian life. Supports the service member through the continuum of care from recovery to rehabilitation and reintegration.
Assists in the development, generation and updating of individualized recovery plans.
Provides guidance and counseling to wounded, injured and disabled service members and their families to ensure all eligible benefits are received and to assist with transition from military to civilian life.
Coordinates access for family members to medical and non-medical care management services including rehabilitation, education, employment-related programs, and disability benefits.
Conducts intake, assessments, makes recommendations and follows-up with eligible individuals.
Networks with community groups to identify resources in local areas. Coordinates support with several outside agencies including military subject matter experts, the Department of Veteran’s Affairs, State Employment Agencies, Educational Liaisons, installation officials, and several other agencies that service these service members including non-profit organizations.
Supplies information on benefits and compensation, legal resources, financial counseling and support.
Participates in Recovery Team Meetings to improve team coordination and delivery of care.
Minimum education and experience:
Bachelor's degree. Four (4) years of additional experience may be considered in lieu of degree.
Two (2) years of related experience.
Ability to access and interpret information on behalf of service members and their families.
Ability to provide support to service members and their families through the entire spectrum of care management, rehabilitation and transition service.
Strong communication skills, to include writing and oral communication.
Knowledge of military protocols and customs.
Occasional travel may be required.
Ability to obtain interim DOD Secret clearance.
Prior military experience with applicable civilian community service experience or experience supporting the Wounded Warrior program.
Experience with wounded, ill and recovering veterans.
Knowledge of support programs and processes provided by Department of Defense, Department of the Navy, Department of Veterans Affairs, Department of Labor and other governmental and non-governmental support agencies.
Knowledge of benefits and experience in assisting individuals access benefits.
Previous military experience or as Spouse of Service Member.
Final DOD Secret clearance.
