Brookdale Healthcare Services is looking for a Home Health Registered Nurse to provide services in the San Antonio, TX area. Join a dynamic team of health care providers focused on health, wellness and prevention for all our patients.

Your responsibilities:

  • Provide care to residents in accordance with physician orders, recognized standards of practice and established company policies and procedures

  • Implement and coordinate the delivery of care in collaboration with physician and resource health care personnel

  • Document all pertinent information regarding nursing care, care plans, observation of the residents" overall condition and behavior, Medicare charting, admission, discharge, and patient teaching

  • Communicate effectively with the residents, families, visitors, members of the health care team, and other disciplines

  • Utilize the nursing process (assessment, planning, implementation, and evaluation) in applying nursing knowledge

Required skills and qualifications:*

  • Graduate from Accredited Nursing School and a minimum of one to two years related experience required

  • Current State RN License

  • Basic CPR/first aid training

  • Home Health experience preferred, but not necessary
