Baylor Scott & White Health Certified Diabetes Educator Job in Waco, Texas
Plans, directs, and provides nutrition services and counseling and education to patients and families. Screens and assesses patient nutritional needs. Provides education for nursing and medical staff, students, and dietetic interns. Provides individualized diabetes self-management education/training to patients and families.
PERFORMS NUTRITIONAL ASSESSMENT TO DETERMINE APPROPRIATE NUTRITIONAL TREATMENT PLAN
*Reviews all necessary medical records to obtain baseline data.
*Interviews patient and/or family in a professional manner to obtain nutritional history.
*Consults with physician or other providers as needed.
*Determines appropriate treatment plan and makes clear and concise recommendations.
*Documents nutritional assessment and recommendations in the patients medical record in an accurate and timely manner.
EVALUATES TREATMENT EFFICACY AND PATIENT PROGRESS TO AID IN ESTABLISHING OR REVISING SPECIFICS OF TREATMENT PLAN
*Creates nutritional plans that are consistent with the physicians prescriptions, directions, and goals.
*Conducts the appropriate and necessary on-going evaluations that will lead to a correct nutritional diagnosis of the patient's problems and progress while maintaining safety and professional standards.
PROVIDES INDIVIDUALIZED DIABETES SELF-MANAGEMENT EDUCATION/TRAINING TO INDIVIDUALS AND GROUPS
*Collects patient assessment data in a collaborative and ongoing manner.
*Collaboratively develops educational goals, learning objectives, and a plan for educational content and teaching methods
*Provides educational interventions that utilize primarily interactive, collaborative, skill-based training methods and maximizes the use of interactive training methods
*Collaboratively develops an individualized follow-up plan with each patient.
*Evaluates effectiveness of educational services provided by measuring attainment of learning objectives.
*Conducts a follow-up assessment upon completion of services.
*Documents assessment data, educational plan, educational services provided and evaluation results in each patients medical records.
*Utilizes a team approach to provide services and collaborates and communicates with team members when needed.
*Identifies when a patients needs are outside the scope of the CDEs practice and expertise, and arranges for additional services to meet needs.
SERVES AS AN EDUCATOR AND RESOURCE PERSON FOR PATIENTS, CARE-GIVERS, MEDICAL AND NURSING STAFF AND COMMUNITY GROUPS
*Provides individual and group patient and/or caregiver education in a clear, concise and professional manner.
*Supervises and coordinates activities of dietetic interns and other students in a manner that promotes efficient and safe operations and provides appropriate development opportunities
*Provides education and serves as a resource to the community in a professional and timely manner.
PARTICIPATES IN PROFESSIONAL DEVELOPMENT, RESEARCH AND PERFORMANCE IMPROVEMENT ACTIVITIES TO IMPROVE AND MAINTAIN REQUIRED QUALIFICATIONS AND STANDARD OF PERFORMANCE.
*Participates regularly in performance improvement activities.
PERFORMS OTHER POSITION APPROPRIATE DUTIES AS REQUIRED IN A COMPETENT, PROFESSIONAL AND COURTEOUS MANNER.
REQUIRED SKILLS, KNOWLEDGE, AND ABILITY:
*In-depth knowledge about current diabetes treatment management
*Ability to develop a collaborative, therapeutic alliance with individuals.
*Basic computer skills
*Must have the ability to read, write, speak and comprehend the English language.
*Must have corrected vision and hearing to normal range.
*Serves as a resource and provides in-service education for health care staff in a professional and timely manner.
*Fulfills continuing education and/or certification requirements to maintain essential qualifications.
*Takes personal responsibility to understand and complete the professional and technical requirements of the position and provide Scott & White with the necessary documentation on such requirements.
