Baylor Scott & White Health HIM ANALYST III in Waco, Texas

The Health Information Management (HIM) Analyst III is responsible for record management responsibilities which include the following: completion of birth registry requirements including Acknowledge of Paternity; release of information requests; document imaging functions to include scanning and quality control; record analysis and completion functions to monitor and assist providers including delinquency procedures; support for coding and transcription to ensure all documentation is complete. The specific functions are assigned depending on software applications used and staff needs at each facility.


  • Answers and directs phone calls appropriately. Opens, sorts and distributes incoming mail to appropriate personnel
  • Completes legal birth certificate and files in accordance with State laws and Hospital policies within five (5) days of birth utilizing State required software application. Completes Acknowledgement of Paternity (AOP) documentation for unmarried parents according to State laws. Updates baby demographic information in computer system with birth certificate name after discharge
  • Processes all requests for patient information to ensure proper authorization for all release of information requests and record review requests while adhering to all State and Federal regulations
  • Receives and/or retrieves, prepares, scans, indexes, performs quality control and archives documents for the purposes of record retention
  • Performs scanning quality reviews/audits, makes needed corrections to include moving/deleting documents or works document correction work queues to correct identified scanning errors
  • Assesses and monitors record deficiencies for all charts, assigns to appropriate provider for completion and assists providers with medical record completion as necessary
  • Researches, troubleshoots, and resolves documentation issues on accounts
  • Performs duties for provider delinquency procedures including monitoring of deficiencies, printing and compiling of reports and provider letters according to Hospital Medical Staff policies and/or Rules and Regulations
