Position: RN Field Nurse - Star KidsReports to: Director of Clinical ProgramsService Area: Parker county and surrounding areas

Job Description/Requirements:

As a RN Field Nurse - Star Kids you will:

  • Engage Members/families in person in member home or community settings to obtain the most accurate information needed to complete the assessment.

  • Call member/family the day before to confirm appointment.

  • Complete the assessment in an electronic medical record file using Harmony.

  • Ensure that documentation is completed, accurate and synched by the end of the day.

  • Travel in assigned territory as needed to complete assessments.

  • Complete SAI, and when prompted will also possibly complete the PCAM, NCAM and MDCP. (To complete the MDCP the you must have current RUG’s certification.)

  • Check schedule on tablet throughout the day and accept assignments.

  • Notify Nurse Manager immediately regarding any problems or issues identified while in the field.

  • Turn in your availability for monthly assignments 1 month prior on the first of every month to the nurse manager.

Requirements of Qualified Candidates

  • RN with valid TX license

  • Bilingual English/Spanish skills

  • Pediatric nursing experience

  • Able to travel locally through territory

  • Able to work 10 hours a week or more
