Located adjacent to the Texas Medical Center, Park Plaza and Plaza Specialty Hospitals offering 444 licensed beds and the convenience of a community hospital with two distinct purposes. Since 1975, Park Plaza Hospital has been offering complete medical services, including surgery as well as outpatient, imaging and rehabilitation. Plaza Specialty Hospital, a hospital within a hospital, has been providing long-term acute care to those needing prolonged hospitalization and recovery since 1994.
Park Plaza/Plaza Specialty Hospitalis currently looking for a Clinical Liaison.
_Responsibilities:_ _ _ * Demonstrates the knowledge and skills necessary to assess data, interpret information and consideration of the age specific needs to the patient. Ensures that assessment form is complete, accurate and legible. Ensures patient and/or family and physician goals are obtained and discussed with attending physician upon screening of patient. Ensures that education and communication are provided to patient/family related to admission, treatment plan and facility services where applicable. * Completes intake referral information and updates referral log ensuring all patients presented for admission meet admission criteria, have appropriate insurance carrier and benefits, have admitting physician and appropriate consulting physicians when applicable. Completes pre-admission process by obtaining all appropriate approvals for patients. * Demonstrates effective communication of patient information to all applicable departments to include assessment copies and verbal communication through flash meetings. Incumbent may also be responsible for admission coordination and intake during critical staffing periods in department. * Receives and dispatches referrals as appropriate. * Works closely with business office to obtain appropriate financial information on patients. * Responds promptly to referrals, adjusting assignments/hours as necessary to achieve maximal customer satisfaction. * Completes pre-admission process by obtaining bed assignments for patients and obtains appropriate approvals. * Maintains up to date knowledge of managed care trends and performs pre-certifications on patients referred to include appropriate negotiation of rates as applicable. * Works closely with Physician Relations staff to plan and implement business development activities such as health fairs, vendor booths, educational programs, luncheons, tours etc.
_Job Requirements:_ _ _ Education:Bachelor's degree in Nursing or Associates degree in healthcare related field (Respiratory, Social Work, Physical Therapy, etc) required.
License/Certification: Licensed/ Certified within your clinical specialty required.
Training and Experience: At least 3 years of clinical experience within specialty. Additional experience in patient intake or a similar role in a hospital or other healthcare setting. Skills and Knowledge Excellent oral and written communication skills. Must be self-motivated. Must be able to travel independently in personal vehicle.
