Position Summary:

Provides linkage service into medical care to both new and previously tested patients who are HIV positive. Advocates on their behalf to decrease service gaps and remove barriers to medical services; helping develop and utilize independent living skills and strategies.

Division of Infectious Diseases:

The Division of Infectious Diseases’ goal is to excel in every aspect of patient care, education, and research related to infectious diseases. We seek to provide the highest quality of patient care with thoughtfulness and compassion. Our division provides inpatient consultation for children with unusual or severe infections, as well as assistance and management of infectious diseases and their consequences. Our general infectious diseases clinic offers outpatient consultation for children with a variety of infectious concerns. We also have a specialty outpatient clinic that provides care to HIV-infected children and adolescents from Houston, Texas and adjacent states.

Position Key Accountabilities:

  1. Provides appropriate outreach and service linkage activities to patients, regardless of their HIV status, with information, referrals and assistance with linkage to medical, mental health, substance abuse and psychosocial services as needed. Assist clients in obtaining needed resources, including bus pass vouchers.

  2. Provides data entry and analytical support for monthly HIV grant reporting requirements, tracks and monitors number of patients who are offered, and receive, HIV tests each month. Tracks all positive HIV patients utilizing computer database programs, enters all required information and close cases as appropriate. Coordinates cases for all appropriate patients to ensure the highest quality care.

  3. Assists HIV-positive patients in overcoming barriers to obtain needed medical care and social services; facilitates and coordinates services between health care providers; helps patients utilize resources, including scheduling appointments and assisting with completion of applications for programs for which they may be eligible, and follows up on pending applications. Motivates patients to be active and engaged participants in their health through clinic and community outreach.

  4. Confers with clinical teams and nurses regarding patient care plans as they relate to linkage to care. Effectively and professionally communicates with all levels of staff, volunteers, and patients.

  5. Accurately tracks and documents communication with patients; calls and sends letters to patients regarding testing due dates and follows up with patients.

  6. Continuously expands knowledge and understanding of community resources, services and programs provided. Schedules outreach/education events in the community and conducts health education workshops at clinic and in the community.

  7. Handles disclosure of HIV-diagnosed partner notification.

  8. Adheres to privacy and information security requirements (HIPAA) and other regulations. Maintains high level of respect and confidentiality in handling patient information at all times.

  9. Performs other duties as assigned.


  • Spanish speaking required.

  • Valid Driver’s License required and must have the ability to drive to different locations for outreach activities.

  • Evenings and weekends occasionally required for outreach.

  • Strong interpersonal communication skills, excellent public speaking abilities.

  • Strong written and verbal English skills.

  • Excellent organizational skills and must be detail-oriented.

  • Experience using Microsoft Excel and PowerPoint preferred.

  • Ability to analyze data, or interest in learning to analyze data.

  • Must have telephone speaking skills and ability to communicate well in a group situation.

  • Ability to multi-task and work cooperatively with others.

  • Ability to perform all tasks and job responsibilities safely without injury to self and others.

  • Demonstrated ability to respond with sensitivity to people of diverse cultures, various lifestyles and diagnoses.

  • Excellent customer services skills.

Minimum Education:

  • Bachelor’s degree from an accredited college or university with a major in social or behavioral science.

Minimum Experience:

  • One (1) year of experience in a clinical field.

Additional Information:

  • Full Time / Non-Exempt

  • Position is located in the Texas Medical Center but will be also require mobility for outreach and services

  • Position reports to Dr. Samuel J. Prater

Physical Requirements:

Exerts up to 20 pounds of force occasionally and/or up to 10 pounds frequently and/or a negligible amount constantly to move objects.
